DeSoto Transitional Care Center (DeSoto TCC) is a 100-bed transitional skilled nursing facility located in DeSoto, Texas, a densely populated and economically thriving community within the Dallas/Fort Worth Metroplex. The Class A, 55,000 square foot facility was developed in 2019 by Madison Marquette in partnership with Global Prime Health, LLC and an associated group of multidisciplinary physicians and operation specialists.
The development is unique in that it is designed and licensed as a skilled nursing facility but serves as a bridge between the hospital and home. The delivery model is a cross between a traditional skilled nursing care facility and an inpatient rehabilitation facility, with the majority of patients staying less than 30 days and a small percentage staying longer, depending on their level of need. Unlike traditional skilled nursing facilities, DeSoto TCC provides specific medical and rehabilitation services inclusive of neurology, orthopedic, cardiopulmonary and wound care, all emphasizing expedited patient recovery.
